You can raise a complaint without creating an account. You will receive a tracking code that lets you follow it and add information later.

What happens to what you write

Your submission is sealed on arrival. Only the Internal Committee for your organisation can open it.
Your identity is never shown in any dashboard, chart or export. Aggregate statistics suppress any group small enough to identify a person.
Every access to your case is recorded in a tamper-evident log, including by administrators.

Intake form arrives in Phase 1

The form is driven by whichever blueprint an organisation has active, so it is built alongside the blueprint engine rather than hardcoded now and rewritten later.
